    Periodontal infrabony defects: Systematic review of healing by defect morphology following regenerative surgery.

    AbstractBackgroundIt is thought that infrabony defect morphology affects the outcome of periodontal regenerative surgery. However, this has not been systematically investigated.AimsTo investigate how well defect morphology is described in papers reporting regenerative therapy of periodontal infrabony defects and to investigate its effect on clinical and radiographic outcomes.Materials and MethodsA search was conducted in 3 electronic databases for publications reporting clinical and radiographic outcomes of periodontal intraā€bony defects after regenerative therapy, divided by defect morphology.ResultsThe initial search resulted in 4487 papers, reduced to 143 after first and second screening. Fifteen of these publications were suitable for a fixedā€effects metaā€analysis. Initial defect depth was found to influence radiographic bone gain 12 months postā€surgery, while narrower angles and increased number of walls influenced both radiographic bone gain and clinical attachment level (CAL) gain at 12 months. These associations seemed to occur irrespective of biomaterials used. Risk of bias ranged from low to high.ConclusionDeeper defects with narrower angles and increased number of walls exhibit improved CAL and radiographic bone gain at 12 months postā€regenerative surgery. More data are needed about other aspects of defect morphology such as extension to buccal/lingual surfaces

    La tesi si propone di esaminare il ruolo e i limiti dell\u2019integrazione del contratto nell\u2019odierno panorama giuridico nazionale ed europeo. Lo studio prende avvio dall\u2019esame delle fonti di eterointegrazione previste dal codice civile, al fine di inquadrarne i limiti e di definire i confini dei poteri attribuiti al giudice, volgendo lo sguardo anche verso altri ordinamenti di Civil law, come quello francese, e di Common Law, come quello inglese, oltre che ai progetti volti all\u2019armonizzazione della legislazione europea. Successivamente ampio spazio \ue8 dedicato al meccanismo di riduzione equitativa della clausola penale e al dibattito, sviluppatosi in dottrina e in giurisprudenza, intorno all\u2019art. 1384 c.c., disposizione che, a seguito di una interpretazione evolutiva, pu\uf2 oggi considerarsi ordinaria e non pi\uf9 eccezionale. In seguito si prende in considerazione l\u2019integrazione nel \u201cnuovo diritto dei contratti\u201d, che si \ue8 affiancato, anche sotto l\u2019impulso della normativa europea, alla disciplina contenuta nel codice civile, consentendone inedite letture. In particolare vengono studiati i contratti dei consumatori e la disciplina della nullit\ue0 delle clausole vessatorie, nonch\ue9 alcuni profili dei contratti tra professionisti, con l\u2019obiettivo di valutare se e in quale misura si possa parlare di evoluzione dell\u2019art. 1374 c.c., con speciale riferimento all\u2019equit\ue0.This thesis aims to examine the role and limits of the integration of the contract in today's National and European juridical scenery. The study starts with the examination of the sources of integration disciplined in the Civil Code, in order to define its boundaries and the confines of the powers attributed to the judge, taking into consideration the Civil law and Common law systems (such as the French and the English ones), as well as the projects aimed at the harmonization of European legislation. Subsequently ample space is devoted to the mechanism of equitable reduction of the penalty clause and to the debate, developed in doctrine and jurisprudence, around the art. 1384 of the Civil Code, which can now be regarded as an ordinary -and no more exceptional- rule, as a result of an evolutionary interpretation. The research continues with a study about the integration of the "New Contract Law", which generated with the model and influence of the European law and is nowadays working alongside the discipline contained in the Civil Code, allowing its new interpretations. In particular, this section of the thesis involves the Consumer Contracts and the rules about the invalidity of unfair clauses, as well as some profiles of Professional Contracts, to assess the evolution of art. 1374 cc, with special reference to equity

    Biologically guided implant therapy: A diagnostic and therapeutic strategy of conservation and preservation based on periodontal staging and grading

    Biologically guided implant therapy is based on the new periodontitis classification system recently released by the American Academy of Periodontology and the European Federation of Periodontology that uses staging and grading for the diagnosis of periodontitis. This paper proposes that periodontitis staging and grading should be used in dental implant therapy as a means to ensure maximum conservation of teeth and maximum preservation of alveolar bone. These biologic principles should guide the treatment planning process and supersede a mechanically based, restoratively driven rationale that should be secondary to the biologic principles of conservation and preservation but part of the collaborative treatment planning process. And treatment alternatives throughout the patientā€™s lifetime should be provided for in case of prosthesis loss due to periĆ¢ implantitis.     Elevated protein kinase C alpha expression may be predictive of tamoxifen treatment failure

    We previously reported that stable transfection of protein kinase C alpha (PKCĪ±) into T47D human breast cancer cells results in tamoxifen (TAM)-resistant tumour growth. Relevance of PKCĪ± expression in clinical specimens was determined by comparing PKCĪ± expression in tumours from patients exhibiting disease recurrence with patients remaining disease-free following TAM treatment. Our results suggest that PKCĪ± expression may predict TAM treatment failure

    Clinical outcomes of periā€implantitis treatment and supportive care: A systematic review

    To report the clinical outcomes for patients with implants treated for peri-implantitis who subsequently received supportive care (supportive peri-implant/periodontal therapy) for at least 3 years. A systematic search of multiple electronic databases, grey literature and hand searching, without language restriction, to identify studies including ā‰„10 patients was constructed. Data and risk of bias were explored qualitatively. Estimated cumulative survival at the implant- and patient-level was pooled with random-effects meta-analysis and explored for publication bias (funnel plot) at different time intervals. The search identified 5,761 studies. Of 83 records selected during screening, 65 were excluded through independent review (kappa = 0.94), with 18 retained for qualitative and 13 of those for quantitative assessments. On average, studies included 26 patients (median, IQR 21-32), with 36 implants (median, IQR 26-45). Study designs (case definitions of peri-implantitis, peri-implantitis treatment, supportive care) and population characteristics (patient, implant and prosthesis characteristics) varied markedly. Data extraction was affected by reduced reporting quality, but over 75% of studies had low risk of bias. Implant survival was 81.73%-100% at 3 years (seven studies), 74.09%-100% at 4 years (three studies), 76.03%-100% at 5 years (four studies) and 69.63%-98.72% at 7 years (two studies). Success and recurrence definitions were reported in five and two studies respectively, were heterogeneous, and those outcomes were unable to be explored quantitatively. Therapy of peri-implantitis followed by regular supportive care resulted in high patient- and implant-level survival in the medium to long term. Favourable results were reported, with clinical improvements and stable peri-implant bone levels in the majority of patients

    This editorial provides clarifications on the application of the Stage and Grade classification of periodontitis.     Multi-stakeholder contribution to the identification of a core outcome set and measurements in implant dentistry (ID-COSM initiative) using the Delphi methodology

    AIM: To obtain input from multiple stakeholders and generate agreement on essential outcomes in implant dentistry using the Delphi methodology and incorporate them into an international consensus defining a core outcome set. MATERIALS AND METHODS: Candidate outcomes in implant dentistry were generated from scientific evidence through five commissioned systematic reviews and from people with lived experience in dental implants (PWLE) through four international focus groups. A steering committee identified stakeholders among representatives from dental professionals, industry-related experts and PWLE. Participants underwent a three-round Delphi survey using a multi-stakeholder approach; they assessed candidate outcomes and additional outcomes identified in the first Delphi round. The process followed the COMET methodology. RESULTS: From the 665 potential outcomes identified in the systematic reviews and 89 in the PWLE focus group, the steering committee selected 100 and organized them into 13 categories, to be included in the first-round questionnaire as candidate outcomes. A total of 99 dental experts, 7 dental-industry-related experts and 17 PWLE participated in the first round, and 11 additional outcomes were added to the second round. There was no attrition between the first and second rounds, where 61 (54.9%) outcomes exceeded the pre-established threshold of agreement. PWLE and experts participated in the third round that applied "a priori" standard filters to distil a list of candidate essential outcomes. CONCLUSION: This Delphi study utilized a standardized, transparent and inclusive methodology and preliminarily validated 13 essential outcomes organized into four core areas. These results informed the final stage of the ID-COSM consensus

    PKCĪ± and ERĪ² Are Associated with Triple-Negative Breast Cancers in African American and Caucasian Patients

    Although the incidence of breast cancer in the United States is higher in Caucasian women compared with African American women, African-American patients have more aggressive disease as characterized by a higher percentage of triple-negative breast cancers (TNBCs), high-grade tumors, and a higher mortality rate. PKCĪ± is a biomarker associated with endocrine resistance and poor prognosis and ERĪ² is emerging as a protective biomarker. Immunohistochemical analysis of ERĪ² and PKCĪ± expression was performed on 198 formalin-fixed paraffin-embedded primary infiltrating ductal carcinomas from 105 African-American and 93 Caucasian patients. PKCĪ± is positively correlated with TNBC in patients of both races and with high tumor grade in African-American patients. Patients with TNBC express less nuclear ERĪ² compared with all other subtypes. We find no difference in frequency or intensity of PKCĪ± or ERĪ² expression between African-American and Caucasian patients. PKCĪ± and ERĪ² are discussed as potential therapeutic targets for the treatment of patients with TNBC
